For cold treatment, you just have to remove the mask from its cover and leave it in the fridge for an hour or in the freezer for 15 minutes.

For hot treatment you can leave it under warm water for 10-15 min or heat in the microwave in 30 second intervals until reaching the desired temperature.

In both cases, you must put the gel mask back in its cover before applying it to the area to be treated.
